Thyroid nodules and HIFU treatment

The thyroid gland is located at the base of the neck and serves to regulate vital processes in the entire body. Formation of nodes in the thyroid gland is due to impaired hormonal function in the gland resulting in structural changes in the cells causing small nodules to form in the thyroid parenchyma.

In most cases, the thyroid nodules do not lead to complaints. They are often accidentally detected during an ultrasound examination. For the most part, the thyroid nodules are benign.

Methods of treatment:

1. Small nodes that do not increase in size over time are only subject to observation

2. Radioactive iodine is used to treat a toxic multiple-nodules thyroid gland

3. Drug therapy is applied when it is necessary to suppress thyroid hyperfunction

4. Surgical treatment is mandatory when:

• a very large node presses the esophagus and trachea, resulting in difficulty breathing and swallowin
• in case of suspicion of a progressive malignant process

* The patient should be aware of the risks of any thyroid surgery because it plays an immense role in the main vital metabolism and metabolic activity of the whole organism, including the brain. After surgical removal of the gland, the patient needs to remain under life-long surveillance and hormone treatment.

The latest technology:

HIFU therapy is a new non-invasive method for removing benign nodules of the cranial gland. High-intensity focused ultrasound achieves ablation only in the cells of the node without affecting the surrounding healthy tissues. HIFU is a very good alternative to invasive surgery because it preserves the organ and has a lower risk of complications. If necessary, the procedure may be repeated.

Parkinson's disease treatment with HIFU in Europe

Parkinson's disease is a progressive neurological disorder which is characterized by a degenerative process in a brain area called substantia nigra. The main disorder is associated with a decrease in the amount of dopamine.  It is an important neurotransmitter involved in the transmission of information between nerve cells. The three main symptoms of the disease are tremor while in a state of rest, stiffness, and slowness of movement. The fourth sign that occurs with the progression of the disease is instability of the posture.  It is possible that gastrointestinal disorders, blood pressure, depression, insomnia and other functional disorders may occur. As the disease progresses, the tremor intensifies to the extent that the patients are unable to perform their daily activities.

Parkinson's disease is considered to be among the incurable diseases. To alleviate the symptoms, a combination of drugs, psychotherapy, physiotherapy and neurosurgery is applied. A new revolutionary method for treating Parkinson's disease is HIFU therapy. Using focussed ultrasound affects a key center deep in the brain, resulting in a stop to the tremor. The procedure is completely non-invasive, harmless, without anesthesia while the patient is fully conscious and the results are monitored in real time. The procedure takes about 4 hours and requires only 2 days of hospitalization. Although at this stage the impact of the therapy is unilateral (the dominant part is chosen), stopping the tremor dramatically improves the patient's quality of life.

Compared to deep brain stimulation, focused ultrasound is a single procedure, and does not require subsequent procedures/visits to replace batteries, repair broken wires, or adjust simulator settings. It also does not involve the collateral damage to healthy tissue or the risk of infections associated with implanting a foreign body.

The ExAblate system manufactured by InSightec is approved in Europe for treating tremor-dominant Parkinson's disease. The countries that apply this method are Spain, Italy, Switzerland and Israel.

Non surgical treatment that keeps their uterus and retains the ability to have future children…

In 2004, the director of our MRI centers heard of this new technology coming out of a company in Israel called Insightec and invited them to give us a lecture on focused ultrasound. I sat in the lecture and couldn’t believe that in my lifetime this would be possible. Our center purchased the system, and we performed the first commercial treatment for uterine fibroids after FDA approval in 2004. Although the director, Dr. Fred Steinberg, was supposed to do the first case, I filled in at the last minute. The treatment was successful, and I was hooked after I saw how the patient walked out of the MRI center one hour after the procedure as if I had done nothing to her!

We have participated in several clinical research projects. We were part of the multicenter trials using focused ultrasound for African American women with fibroids, and I have also completed some of my own clinical research with fibroids. We also participated in the clinical trial that led to the Insightec system being approved by the FDA to reduce pain from bone metastases.

From 2004 to 2008, there was a lot of buzz around focused ultrasound for fibroids. We had many women wanting to participate in the early research studies, partly because there was no cost, and because women wanted a non-surgical option.

Treatment volume began to slow during the start of the recession in 2008. We also did not have insurance coverage then, which continues to be a challenge today. Despite FDA approval and many clinical studies demonstrating the efficacy of focused ultrasound for fibroids, payers still want a long term comparative study before they will reimburse for the procedure. A study to generate this data is prohibitively expensive and hasn’t been conducted, so we don’t have widespread insurance coverage in the US. Without this coverage, only a few sites across the country have been offering the procedure.

HIFU Treatment in Bulgaria

Despite these challenges, we recognize that the benefits of focused ultrasound are so compelling for patients that they would be willing to pay for the treatment. So the Fibroid Education Center was created to empower women to make informed decisions and make the procedure more accessible. We are aiming to turn the tide by generating awareness of focused ultrasound, increasing the number of sites offering the procedure, and ultimately, leading insurance companies to recognize the incredible value of this option for women. I am hopeful because some payers have recently changed their policies to cover focused ultrasound for fibroids.

When I think of the millions of women suffering from their fibroids, I know that many of them want a solution that doesn’t involve surgery such as a hysterectomy or myomectomy. As a non-surgical alternative, focused ultrasound can improve their symptoms, minimize complications often associated with other fibroid treatments, and enable them to return to work and their activities of daily living much faster. In 2015, the FDA changed the labeling so Curawave can also be used in women who wish to maintain their uterus for future fertility. Over the past few months, I have treated many women in their early 30’s who are so grateful to have a nonsurgical treatment that keeps their uterus and retains the ability to have future children!

Το ρολόι γύρισε πίσω για την Kimberly

Για την Kimberly Spletter, οι βόλτες με το ποδήλατο ήταν πάντα μια πηγή χαλάρωσης, μέχρι που διαγνώστηκε με τη νόσο του Πάρκινσον. Δεν μπορούσε πλέον με ασφάλεια να ισορροπήσει πάνω του και αντί γι'αυτό έπρεπε να στραφεί σε ένα σταθμευμένο ποδήλατο. Οι ανεξέλεγκτες κινήσεις που σχετίζονται με την ασθένεια φέρνουν σε κίνδυνο ακόμη και την ικανότητά της να περπατήσει.

Τα συμπτώματα άρχισαν πριν από έξι χρόνια, στην ηλικία των 44, ως ήπια δυστονία στο αριστερό της πόδι. Τα δάχτυλα των ποδιών της άρχισαν να κάμπτονται προς τα κάτω, και σύντομα τα δάχτυλά στο αριστερό της χέρι άρχισαν να μουδιάζουν. Η Kimberly είχε διαγνωστεί με τη νόσο του Πάρκινσον και άρχισε να παίρνει Levodopa για διαχείριση των συμπτωμάτων της. Δυστυχώς, η φαρμακευτική αγωγή συχνά παράγει τις δικές της εξουθενωτικές παρενέργειες  και μετά από μόλις έξι μήνες η Kimberly άρχισε να παρατηρείται δυσκινησία ή ακούσιες κινήσεις και τρέμουλο. Η διαδικασία θα είναι μη επεμβατική και θα μπορούσε να προσφέρει ανακούφιση από τα συμπτώματα, τα οποία θα άλλαζαν τη ζωή της Kimberly. Αλλά η θεραπεία ήταν πειραματική. Η Kimberly θα ήταν μία από τους πρώτους ασθενείς στις ΗΠΑ που θα την δοκίμαζαν.

«Έκανα την έρευνά μου, γιατί ήθελα να είμαι ενημερωμένη όσο το δυνατόν καλύτερα… Πάνω απ 'όλα, η οικογένεια μου και οι φίλοι μου με στήριξαν πλήρως, έτσι ήξερα ότι είχα πάρει τη σωστή απόφαση.», λέει η Kimbery.

Κατά τη διαδικασία η Kimberly  ήταν ξύπνια, είχε μόνο μια αίσθηση θερμότητας, λίγη ναυτία και πίεση από το πλαίσιο που είχε τοποθετηθεί στο κεφάλι της. Αλλά τα θετικά αποτελέσματα ήταν άμεσα. Μετά την ολοκλήρωση της θεραπείας με εστιασμένο υπέρηχο υψηλής έντασης, η οποία κράτησε τέσσερεις ώρες, η Kimberly ήταν σε θέση να περπατήσει μόνη της μέχρι το δωμάτιο.

Μέρες μετά τη θεραπεία, η Kimberly είναι ενθουσιασμένη. «Είχα ήπιο πονοκέφαλο, αλλά έτρεξα, περπάτησα και έκανα ποδηλασία. Το ρολόι γύρισε πίσω για μένα.»

Αν και κατανοεί πολύ καλά ότι υπάρχει πάντα ο κίνδυνος που συνδέεται με τις ιατρικές καινοτομίες, η Kimberly ενθαρρύνει και άλλους ανθρώπους να βοηθήσουν την έρευνα να πάει μπροστά. «Είναι η ελπίδα μου ότι οι άνθρωποι θα συμμετάσχουν σε κλινικές δοκιμές. Νομίζω ότι ο μόνος τρόπος να βρούμε μια θεραπεία είναι με τη δοκιμή νέων ιδεών »…

World's First Focused Ultrasound Clinical Trial for Epilepsy Begins

Researchers at the University of Virginia (UVA) are starting the first clinical trial in the world using focused ultrasound to treat patients with epilepsy. The study, supported by the Focused Ultrasound Foundation in collaboration with the Epilepsy Foundation, will assess the feasibility, safety and initial effectiveness of focused ultrasound to non-invasively destroy (ablate) diseased brain tissue that causes seizures.

The study is now recruiting up to 15 adult patients with a range of rare deep brain lesions that produce debilitating seizures that often do not respond to medications.  It is expected that most patients in the study will have benign tumors in the hypothalamus, which can lead to frequent seizures with outbursts of spontaneous laughing, giggling, crying or grunting; developmental delays and/or precocious puberty. The disorder can progress as patients age, leading to increased cognitive decline and severe behavior problems.

Current options for patients whose seizures do not respond to medication include surgery or laser thermal ablation, both which may involve multiple risks or complications, such as damage to normal brain, infection or hemorrhage.  The only currently available non-invasive method is radiosurgery with the gamma knife, which may take months to show effect, is not image guided and involves risks of complications from radiation.

“Intractable epilepsy, especially this type, can be devastating and existing therapies have risks and are not uniformly effective, so many patients are in desperate need of better therapies,” says principal investigator Nathan Fountain, MD, professor of neurology at UVA. “If we are successful in destroying the lesions without complications, then this will be an important step on the path toward developing focused ultrasound to enable patients to have freedom from seizures without the risks of open surgery.”

Targeted drugs at a nano-scale

Pharmaceutical scientist Gert Storm works on the design, research and testing of targeted nanomedicines. These are tiny balls that can be transported to the right place in the body, helping doctors to dose medicine more efficiently and preventing the drug from damaging other parts of the body. "A drug that combats kidney problems should not end up at the lungs."

If you have a headache, you take paracetamol or ibuprofen. After about twenty minutes, your headache will start to subside and it will eventually disappear. But how does a painkiller know that the pain is in your head? And can we control that?

Porous vessel

"A drug enters your bloodstream via an injection or pill," explains biopharmacist Gert Storm. "Let's first make a distinction between a drug and its active substance. The active substance, also known as an active pharmaceutical ingredient (API), the pharmacologically active drug molecule, is the substance that needs to cure us. The drug or medicine is the entire drink, pill or injection."

"The interior of a blood vessel is lined with cells that make the  vessel wall  almost entirely non-porous, but a local infection can make the vessel lining a little bit more porous in that area," says Storm. " However, the drug does not 'know' where to go and only ends up at the right place in only low amounts because our heart pumps blood through our body. Chances are small that the API will eventually end up at the right place. And the substance also tends to leave the body quickly, in urine for instance, so that we have often to start off with a high dosage that is administered frequently.

More efficient dosing

Such intensive dosing schedules are especially unfortunate for medicine as they can lead to severe side effects. So Storm and his colleagues conduct research into Drug Targeting. "We want to guide the drugs to the right 'address' in the body, thereby making high dosages redundant and increasing the efficacy of the medicine without burdening the body parts which do not need it," he explains.  " We are developing medicines that go straight to the source of the problem, where they need to work." One way to do that is to exploit the more porous vessel walls which are often present in diseased areas.

Exploding nanoballs

Storm explains that they work with tiny (‘nano’) balls made of fats or polymers. "We put the API into the balls. There are already fat globules present in our body and are therefore a natural way to transport substances" he explains. "A nanoball will be able to go through the porous vessel wall and is less likely to end up in urine than the API itself"

Storm works at the UMC one day a week. "We work with temperature-sensitive nanomedicines, which are globules that burst open at a specific temperature and then completely release the drug in seconds. We mainly use them to combat tumours. In an MRI scanner, we can identify the precise location of a tumour. We then use ultrasound to heat the tumour  to 42°C, which is exactly the temperature at which the globules are triggered to burst open. The API is delivered at the right place in the right way, and high dosages indusing side effects are  therefore not needed. Other organs, which may be susceptible to the substance, will not be damaged either, as the ultrasound waves are really focussed on the tumour area only."

Full integration

"Right now, we are mainly conducting such ‘triggered drug release’ research with the aim to treat tumours. I think that in the near future we will also be able to expand this technique to other symptoms, such as chronic inflammation and other conditions that are easy to reach with ultrasound. And although we still have to overcome some hurdles, I believe that this treatment method can become a clinical reality within a very short time frame. The first clinical study in breast cancer patients is expected to start this year."
